An outbreak of vaccine-related polio cases has been reported in war-torn Syria, the World Health Organization announced Thursday — another hurdle for a nearly 30-year eradication effort that has seen more than its share of setbacks.
Two children in the Deir-Ez-Zor governorate in eastern Syria have been paralyzed by the vaccine viruses. The virus has been recovered from a third child who was not paralyzed. All signs point to vaccine viruses that are circulating in the area.

Immunized People Getting Whooping Cough
Most of the people who got whooping cough in San Diego County so far this year were up to date with their immunizations, according to county data.
Of the 621 people who contracted the illness, 85 percent had all their preventative shots — calling into question the efficacy of the vaccine.
“Pertussis vaccines offer high levels of protection within the first year of completing vaccinations,” said Dr. Wilma Wooten, a San Diego county public health officer.
“But then the protection decreases over time,” Wooten said.
KPBS and inewsource first raised questions about how well the vaccine worked in 2010, after its statewide analysis of the data showed the majority of people who got the disease were already vaccinated.

Eight HCWs were notified as measles cases; 6 were vaccinated with measles vaccine twice, 1 was vaccinated once, and 1 was unvaccinated. All 6 twice-vaccinated cases had high avidity and PRN titers. None reported severe measles or onward transmission. Two of 4 investigated twice-vaccinated cases had pre-illness PRN titers of >120 mIU/mL. Among 106 potentially exposed HCWs, the estimated effectiveness of 2 doses of measles vaccine was 52% (95% confidence interval [CI], −207%–93%).

Conclusions
The effects of whole-cell pertussis vaccine wane after 5 to 10 years, and infection in a vaccinated person causes nonspecific symptoms (3-7). Vaccinated adolescents and adults may serve as reservoirs for silent infection and become potential transmitters to unprotected infants (3-11). The whole-cell vaccine for pertussis is protective only against clinical disease, not against infection (15-17). Therefore, even young, recently vaccinated children may serve as reservoirs and potential transmitters of infection.

However, on rare occasions, an excreted vaccine-virus can continue to circulate within an environment for a longer than usual period while undergoing genetic changes. In such cases, the virus may morph into a form that can cause paralysis -- just as the wild strain of poliovirus can.

The spread of whooping cough is most easily prevented by receiving the DTaP shot. However – and this is particularly troubling – all 13 of the people confirmed to have Pertussis had gotten the vaccine.

Polio outbreak confirmed in Papua New Guinea
A polio outbreak has been confirmed in Morobe Province of Papua New Guinea, the World Health Organization reported Monday.
Papua New Guinea had been certified polio-free since 2000, along with the rest of WHO's Western Pacific Region.
The virus was suspected as the cause of lower limb weakness in a 6-year-old boy in Morobe Province in April; in May, the diagnosis was confirmed.
