Some of the websites included base preparation and rebar and others did not even make mention of it. It would seem that the information is all over the place. In the end what's important is that the price you're looking at is defined and you know what you're going to be getting. Some contractors seem to focus on a price "per square foot" that is not all-inclusive and so some of the services included in their per square foot price might not be as inclusive as what you get from other contractors.
